What do the three numbers on the bags and boxes of fertilizer mean? And what does each one do?

They represent the percentages of plant nutrients by weight. Just remember 'up, down and all around.' The first number is how much nitrogen is in the mix. Nitrogen makes grass and plants grow and become greener. The second number is how much phosphorous there is. Phosphorous stimulates root and seeding development. The third number is how much potassium there is. Potassium promotes disease and drought tolerance. These numbers are always expressed in the same order.
